What to Bring to Your First Visit

Coming prepared helps us make the most of your time together. Please bring:

  • A photo ID and your current insurance card
  • Any current or previous medical records, including recent lab results and imaging
  • A complete list of your current medications, vitamins, and supplements, including dosages
  • A list of any allergies
  • Your questions and goals for the visit

New patient paperwork must be submitted at least 24 hours before your appointment so our team can process and review it in advance.

What to Expect at Your Appointment

Your first primary care visit typically includes a thorough review of your health and family history, a discussion of your symptoms and goals, and a physical examination. If lab work is needed, we can draw it right in our office through our lab services. Together, we’ll create a plan that may include preventive screenings, chronic disease management, women’s health or men’s health services, and referrals to specialists when needed.

For cosmetic consultations, your provider will evaluate your skin and aesthetic goals, review your medical history, and recommend a personalized plan, which may include Botox, dermal fillers, Morpheus8, or specialty facials. You can preview real results in our Before & After gallery.

Insurance, Payment, and Deposits

We accept many major insurance plans for primary care services and welcome self-pay patients. Review our Insurance page or call us to verify your coverage. For elective treatments, we offer flexible payment plans, including Cherry, CareCredit, and Affirm. Questions about a bill can be directed to our team through our Billing page.

Please note that deposits are required for cosmetic, laser, aesthetic, and certain medical procedure appointments, and we have a 48-hour cancellation policy. Full details are available on our Office Policies page.
