What to Bring to a DOT Physical
Commercial drivers should bring a complete list of medications, including dosages, along with the names and addresses of their doctors. To save time, we recommend completing the health history questionnaire before arriving. Drivers with certain medical conditions should also bring:
- Eyeglasses, contacts, or hearing aids for vision or hearing problems
- Recent hemoglobin A1C results and blood sugar logs for drivers with diabetes
- A letter from your cardiologist outlining your medical history, current medications, and clearance to work for drivers with heart conditions
What the DOT Physical Covers
1. Vision
Drivers must have at least 20/40 acuity in each eye with or without correction and at least 70 degrees of peripheral vision in the horizontal meridian in each eye.
2. Hearing
Drivers must be able to perceive a “forced whisper” at a distance of five feet or less, with or without a hearing aid, which equates to an average hearing loss in the better ear of less than 40 dB.
3. Blood Pressure and Pulse
The medical examiner checks blood pressure and pulse to look for high blood pressure and irregular heartbeats.
4. Urinalysis
A urinalysis is required to look for signs of underlying conditions such as diabetes. Testing is completed through our in-office lab services.
5. Physical Examination
The physical exam covers a dozen categories:
- General appearance
- Eyes (cataracts, glaucoma, macular degeneration)
- Ears (scarring or perforation of the eardrum)
- Mouth and throat (problems breathing or swallowing)
- Heart (murmurs, extra sounds, pacemaker)
- Lungs and chest, not including breast exam
- Abdomen and viscera
- Vascular system (abnormal pulse, carotid, varicose veins)
- Genito-urinary (hernias)
- Extremities
- Spine and musculoskeletal system
- Neurological (equilibrium, coordination, speech, reflexes)
A DOT physical can only be completed by an FMCSA-certified medical examiner, who determines whether a driver meets all requirements.
